Job Summary:
The coordinator provides overall team leadership and ensures the effective delivery of harm reduction and outreach services across the regions. This role oversees day-to-day operations, including Case Manager services, and supports clients by facilitating optimal care through a range of assistance and resources. In collaboration with the Manager, the Coordinator develops and maintains relationships with network partners, represents LOFT at system tables, and ensures staff including partner agency staff are informed about program operations and documentation requirements for reporting purposes. The coordinator is also responsible for guiding staff, maintaining service quality, and fostering strong relationships with individuals who are unhoused, at risk of homelessness, street-involved, or experiencing mental health and substance use challenges.

York Region/Street Outreach & Services Network: The program provides a mobile van service that operates seven days a week from 1:00pm – 9:00pm, except for statutory holidays in York Region. Using a harm reduction approach, the program provides services to residents of York Region experiencing homelessness and those living at low and moderate-income levels. Clients are able to receive support from the van for basic needs and life-saving services in a manner that fosters self-worth and dignity.

What You Will Do:

  • Oversee all aspects of the site management of the Service. This includes managing all health and safety requirements and general maintenance and security.
  • Promote personal empowerment through personalized programming, coaching, mentoring and support;
  • Advocate in navigating the social services and health care systems to ensure accessibility and availability of services;
  • Mediate issues included but not limited to clients, agencies etc. with conflict resolution
  • Provide leadership and support to direct service staff, including scheduling, training, performance management, and recruitment.
  • Develop and implement operating procedures for the site and ensuring compliance with established standards including but not limited to property standards, fire codes and health and safety etc.;
  • maintain a general awareness of the daily activities and client issues and identifying staffing issues;
  •  Responsible for coordinating the acquisition, use, and maintenance of site equipment and supplies, managing repair requests
  • maintain accurate service records and incident reports, and overseeing petty cash handling.
  • Participate in the regular service planning processes including but not limited to service plan, operating plan, client satisfaction feedback process, quality improvement and other research activities and program evaluation report etc;
  • identify gaps in service and recommending solutions, participating in meetings, planning and evaluation initiatives, committees, events and activities;
  • Organize and participate in client meetings or other group activities which promote the well being of the residents and the smooth operation of the service;
  • Provide other related assistance as required by the Program Manager

What You Bring:

  • A degree/diploma in health care/social services or other related field
  • At least 5 years relevant community support service experience in the mental health sector
  • Excellent understanding and implementation of harm reduction and psychosocial rehabilitation principles as acquired through a post secondary education,
  • Knowledge of the issues faced by adults with special needs and of a marginalized client group.
  • Experience in program development, planning and evaluation.
  • An understanding of the therapeutic relationship.
  • Experience in conducting meetings or facilitating groups, good analytical, time management, and organizational skills to co-ordinate and follow-up on issues or activities.
  • Excellent interpersonal, communication and counselling skills are required
  • Advocacy skills with knowledge of local community agencies and other professional services available for the client group 
  • An ability to establish effective relationships with service providers.
  • Good intake assessment, crisis intervention, advocacy and problem solving skills.
  • Computer literacy sufficient to produce and maintain files, records or statistical information.
  • A valid Ontario driver’s license is a must.

Work Environment:

  • Frequent driving, bending, kneeling and reaching, combined hours of sitting and standing while using office equipment and computers, light lifting of supplies and materials.
  • Lifting to 30 pounds, exposure to elements when working mobile outreach shifts, sitting and/or driving for extended periods of time,
  • May be required to be on-call as emergency contact for after hours, on a rotating schedule.
  • Staff will work in the community and may be exposed to secondhand smoke
