To help fulfill its mission, LOFT is hiring a Community Clinician – Occupational Therapist.




Job Summary

The Community Clinician – Occupational Therapist is part of an interdisciplinary team that provides consultation, direct care, capacity enhancement and support to older adults (and their partners in care), living in the community; within the geographical boundaries of York Region and South Simcoe. The Community Clinician is expected to ensure the safety and security of the client(s) environment, to practice in accordance with their professional College, follow professional standards, the policies and procedures as defined by LOFT Community Services. The Community Clinician will be responsible for working with individuals from diverse backgrounds, contributing to Integrated Psychogeriatric Outreach Program development, and providing support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment. The Community Clinician is directly responsible for developing and articulating strategies for the management of recovery, psychosocial rehabilitation and coordination of care using information collected through assessments and collaboration with the integrated care team. Additionally, this role will liaise with community agencies, other practitioners, and key partners, including care partners, to coordinate care for clients.


The Integrated Psychogeriatric Outreach Program delivers support by an interdisciplinary team of Clinicians to older adults living in the community within York Region/South Simcoe who are experiencing a new or functional decline related to mental health, substance use and/or cognitive challenges. The IPOP Team is directly responsible for developing and articulating a treatment plan for the management of recovery, psychosocial rehabilitation and coordination of care using information collected through a comprehensive psychogeriatric assessment and collaboration with the integrated care team members. This is delivered through home visits and access to Geriatric Psychiatry using OTN.




What You Will Do
  • Master's degree in an allied health service such as Social Work, Occupational Therapy and/or Nursing combined with three (3) years of post-graduate work experience in provision of clinical psychogeriatric services;
  • Conduct comprehensive psychogeriatric assessments for older adults referred to IPOP;
  • System/resource navigation and case management coordinating care for patients as needed;
  • Liaise, collaborate and coordinate with community/healthcare providers, primary care, pharmacists, and partners within Integrated Psychogeriatric Outreach Program to support implementation of clinical plan;
  • Develop short-term therapeutic relationships to foster coping and adaptation to health promoting strategies or chronic conditions.
  • Identify changes in appearance/behaviours, reporting physical and mental health concerns, unusual or exceptional occurrences and behaviour;
  • Provide support with behaviours arising from cognitive issues, and contacting appropriate emergency services in crisis situations;
  • Monitor client's symptoms and initiate interventions to facilitate stabilization when required;
  • Maintain all records of service and ensuring supplies and equipment are readily available;
  • Conduct mandatory assessments and provide timely and accurate record keeping and reports as required;
  • Assists with application of behaviour management strategies under the guidance of the geriatric psychiatrist working with the Integrated Psychogeriatric Outreach Program;
  • Develop respectful and trusting relationships with clients using recovery and psychosocial rehabilitation principles;
  • Maintain strict confidentiality and follow all other required policies and procedures.



What You Bring
Must Haves
  • Master's degree in an allied health service such as Social Work, Occupational Therapy and/or Nursing combined with three (3) years of post-graduate work experience in provision of clinical psychogeriatric services;
  • Registration with respective College (Ontario College of Social Workers and Social Service Workers (OCSWSSW), College of Nurses of Ontario (CNO), College of Occupational Therapists of Ontario (COTO), etc.) in good standing;
  • Knowledge of resources available to seniors with mental health issues (i.e. dementia and Alzheimer's) and experience navigating the available system to support patients and families/care partners;
  • Knowledge and experience working with relevant legislation (i.e. SOM Act, POA, Consent and Capacity, etc.);
  • Experience and ability to collaborate effectively with other members of inter-professional care teams, clients, family members and community team members;
  • Excellent assessment skills, including demonstrated knowledge of crisis intervention skills;
  • Excellent oral and written communications skills;
  • An understanding of recovery and psychosocial rehabilitation principles and the therapeutic relationship;
  • Well-developed problem solving abilities and interpersonal skills;
  • Self-directed, results oriented and capable of working independently as well as in a team environment;
  • Excellent critical thinking skills and sound clinical decision making;
  • Demonstrated experience and skills in use of computer technology for clinical care;
  • Demonstrates awareness and understanding in working with individuals from different cultures;
  • Demonstrates initiative and creativity in the workplace;
  • A positive attendance record;
  • Criminal Records Check and Vulnerable Sector Screening;
  • Valid Driver's License and Vehicle required as services will be provided across York Region and South Simcoe
