Job Summary
As the Registered Practical Nurse, you will be part of an interdisciplinary team, who is expected to ensure the safety and security of the resident environment, to practice in accordance with College of Nurses of Ontario professional standards, and the policies and procedures as defined by LOFT Community Services. The RPN will be responsible for working with individuals from diverse backgrounds, contributing to Program development, participating in clinical supervision; and providing support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment. The RPN is directly responsible for developing and articulating strategies for the management of recovery and psychosocial rehabilitation using information collected through assessments and collaboration with the PSWs. Additionally, you will liaise with community agencies, other practitioners, and key partners to coordinate care for clients.
LOFT at Kingston Road is a 70-unit program for seniors facing complex challenges who are able to live safely in the community with access to subsidized housing and enhanced assisted living services. Kingston Road is designed to responds to each resident’s needs, so they can live safely and in good health in the community as they await for permanent housing and support, whether the permanent housing is within Kingston Road or in other programs of their choice.
What You Will Do
- Provides direct nursing care with medications as defined by the College of Nursing Practice including –
- administration of medications through oral, subcutaneous, topical, natural body orifices, intracutaneous, tubing routes
- transcribing medical orders as given by physicians in writing or by telephone
- administration of routine medications including narcotics
- administration of routine p.r.n. medications, effect of such and identify when the order needs to be adjusted.
- maintains security of medication system and storage
- orders medications as required and maintains the MAR system.
- follows all policy and procedures as outlined by the Pharmacy.
- Identify changes in appearance/behaviours, reporting physical and mental health concerns, unusual or exceptional occurrences and behaviour.
- Participate in service planning and development of personal goals for clients to transition to their permanent discharge location;
- Provide support with behaviours arising from cognitive issues, intervening in conflicts between residents, and contacting appropriate emergency services in crisis situations;
- Maintain all records of service and ensuring supplies and equipment are readily available;
- Conduct mandatory assessments and provide timely and accurate record keeping and reports as required;
- Ensure proper care is rendered in a safe and clean environment;
- Liaise with physicians, pharmacists and other health care providers regarding client care;
- Provide advice, coaching and support to staff regarding medical/healthcare questions and concerns;
- Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles;
- Maintain strict confidentiality and follow all other required policies and procedures;
- Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on LOFT Community Services;
- Liaise, collaborate and coordinate with other professionals, service providers and partners within the Program;
- Follows standard infection control practices and enhanced precautions as instructed;
- Effectively participate as a member of the LOFT Team; attend staff/team meetings, client care conferences, other meetings etc. as required;
- Serve as back up to Personal Support Workers, as required;
- Attend and actively participate in all required training;
- Complete other duties, from time to time, as assigned by the Director;

Must Haves
- Currently registered with the College of Nurses of Ontario;
- Graduate of a recognized program for Registered Practical Nurse
- Medication Administration Certification or completion of an RPN education preparation program that includes medication administration
- Experience with vulnerable older adults and seniors with serious mental illness, cognitive, addiction and responsive behavioural challenges who have been longer term hospital patients;
- First Aid and CPR certification;
- Knowledge of social services and community resources and supports for adults and seniors;
- Criminal Records Check and Vulnerable Sector Screening
- An understanding of recovery and psychosocial rehabilitation principles and the therapeutic relationship;
- Well-developed problem-solving abilities and interpersonal skills;
- Self-directed, results oriented and capable of working independently as well as in a team environment
- Physically able to perform demanding body mechanics (i.e. lifting, transferring, Hoyer, etc.)
- Counselling, responding and assessment skills;
- Good oral and written communication skills and computer literacy, including MS Word and Excel;
- Ability to collaborate effectively with other members of inter-professional care teams, clients, family members and community team members;
Relief position offering a flexibility schedule to cover all 3 shifts, 7 days a week. Physically able to perform demanding body mechanics i.e. lifting, transferring, Hoyer, etc.)
