To help fulfill its mission, LOFT is hiring a Relief - Registered Nurse.



JOB SUMMARY
As a member of an interdisciplinary team, you will be responsible for overall client care/service delivery while ensuring the safety and security of the client environment, with adherence to College of Nurses of Ontario professional standards, and the policies and procedures as defined by LOFT Community Services The Registered nurse coordinates the day to day operations at the residence and supervises team members including but not limited to Registered Practical Nurses and Personal Support Workers in coordination with the Program Manager.  The RN will be responsible for participating in clinical supervision; and providing support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment.


What You Will Do
 
  • Oversees the overall nursing care delivery of client, in adherence with College of Nurses professional standards and LOFT policies and procedures.
  • Provides supervision and leadership to care team (RPN and PSW)
  • Conduct clinical assessments and develop nursing care plans for those served by the Back to Home Penetanguishene program.
  • Monitor RPNs’ nursing practice in collaboration with LOFT Program Director and provide, in specific cases, direct nursing care with medications as defined by the College of Nursing Practice including:
  • Administration of medications through oral, subcutaneous, topical, natural body orifices, intracutaneous, tubing routes
  • Transcribing medical orders as given by physicians in writing or by telephone
  • Administration of routine medications including narcotics
  • Administration of routine p.r.n. medications, effect of such and identify when the order needs to be adjusted
  • Maintains security of medication system and storage
  • Orders medications as required and maintains MAR system
  • Follows all policy and procedures as outlined by the Pharmacy
  • Assign and monitor delegation activities between RPNs and PSWs.
  • Provide training and supervision to PSWs in the performance of delegated and controlled acts for clients where direct RN involvement is most appropriate.
  • Provide advice, coaching and support to staff regarding medical/healthcare questions and concerns where appropriate.
  • Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles.
  • Maintain strict confidentiality and follow all other required policies and procedures.
  • Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on LOFT Community Services.
  • Liaise, collaborate and coordinate with other professionals, service providers and partners within Enhanced Specialized Assisted Living Program


WHAT YOU BRING:

Must Haves
  • Registration as a Registered Nurse (RN) in good standing with the College of Nurses of Ontario, RNAO member.
  • University undergraduate degree in Nursing.
  • Minimum of three years’ experience in home and community-based nursing.
  • Experience with vulnerable older adults and seniors with serious mental illness, cognitive, addiction and responsive behavioural challenges who have been longer term hospital patients.
  • An understanding of recovery and psychosocial rehabilitation principles and the therapeutic relationship.
  • Experience delegating controlled nursing acts (e.g. G-tubes, wound care, diabetes management, mechanical lifts, etc.) to unregulated health care providers.
  • Demonstrated ability to use knowledge transfer techniques to support practice delegation and good understanding of learning needs of unregulated care providers and interdisciplinary point of care providers.
  • Strong clinical practices with ability to champion evidence-based best practices.
  • Demonstrated ability to communicate effectively and problem-solve complex clinical situations.
  • Clear general understanding of the continuum of care in the community sector, the Ontario health system and knowledge of social services and community resources and supports for adults and seniors
  • Able to recognize safety issues, follow safe work practices, advocate for client care and safety concerns.
  • First Aid and CPR certification.
  • Self-directed, results oriented and capable of working independently as well as in a team environment.
  • Ability to collaborate with other members of circle of care to solve problems and meet client needs.
  • Demonstrated commitment to modelling lifelong learning and professional growth.
  • Good oral and written skills.
  • Excellent computer skills.
  • Ability to work effectively in a team environment.
  • Valid drivers’ license and access to own vehicle is required

  • Understanding of cultural competency and anti-oppression framework.
  • Reflective of a multi-barriered group, as serviced by the program, a strong asset.
  • Canadian Nurses’ Association certification (Psychiatric and Mental health) an asset
