Who We Are:
LOFT Community Services is a unique and dynamic charitable organization that supports people living with complex issues such as mental and physical health challenges, substance use challenges, poverty and homelessness. Serving 19,534 annually and providing 1,839 units of supportive housing last year, LOFT is one of Ontario’s largest mental health service providers of its kind. LOFT is also recognized as a Nonprofit Employer of Choice™ Award (NEOC) recipient.

Program Summary:
The McEwan Case Management Program provides community-based service navigation, accompaniment, referral, and advocacy services to individuals who are living or at risk of HIV, have experienced and/or are experiencing homelessness or precarious housing, or identify as trans or non-binary and are not adequately connected to health services. The team is comprised of three services: Addictions Supportive Housing (ASH), Mental Health Case Management, and Positive Service Coordination (PSC).

The program provides holistic case management services to individuals staying in a range of shelter, treatment, hospital and detox, and transitional and permanent housing environments; living with complex mental and physical health challenges, substance use barriers, and cognitive impairments; are incarcerated or have experience of incarceration; and present with other support needs. As such, the program aims to provide timely interventions to access and advocate for health care and community-based services, motivate individual goal-setting through collaborative and empowering methods, and reduce isolation through building connections to safe community.
