About the Program:
Assisted Living- Dunn is a 24/7 program that provides specialized assisted living supports to vulnerable older adults and seniors with complex physical health challenges, mental health challenges, addiction or cognitive impairment. Assisted Living - Dunn responds to resident’s needs, so that they can live safely and in good health in the community as they age. Through our care, we aim to postpone for as long as possible, or eliminate, any need for institutional care. The program serves 70 clients living at program sites, as well as an additional 28 clients through Hub and Spoke services in the wider community. The program supports clients living independently in the community with support and dignity. PSW, Case Manager and Life enrichment supports are provided to assist clients at home and in the community.

What You Will Do:
  • Review client background information and interview clients to obtain case history and prepare intake reports
  • Assess clients’ skills, strengths, and support needs
  • Assist clients in exploring options and developing action plans while providing ongoing support and assistance
  • Assess and investigate eligibility for social benefits
  • Refer clients to social services and community resources, including legal, medical, financial, housing, employment, transportation, and daycare supports
  • Counsel clients living in group homes and halfway houses and assist with pre-release and release planning
  • Participate in the selection and admission of clients into appropriate programs
  • Implement life skills workshops, substance abuse treatment programs, behaviour management programs, life enrichment programs, senior services programs, and other community/social service initiatives
  • Meet with clients to assess progress, provide support, and discuss challenges or barriers
  • Assist in evaluating treatment program effectiveness by tracking clients’ behavioural changes and responses to interventions
  • Advise and support recipients of social assistance and pensions
  • Provide crisis intervention and emergency shelter services
  • Implement and coordinate the delivery of specialized community services
  • Maintain communication with social service agencies and health care providers to support client progress and continuity of care
  • Maintain program statistics and records for evaluation, reporting, and research purposes
What You Bring:
  • Must be a youth ages between 15 to 30 years of age and eligible to participate in the Canada Summer Jobs program.
  • Be a Canadian Citizen, permanent resident, or someone to whom Refugee Status has been granted/conferred.
  • Have a valid SIN
